Abstract
تناولنا في دراستنا الموسومة بـ "الخصائص المعجمية والدلالية في خطاب المصابين بالحبسة: دراسة لسانية عصبية" العلاقةَ التفاعلية بين نظام اللغة المعرفي والبنية العصبية للدماغ، حيث ركزنا على رصد وتفسير الانحرافات اللغوية الناتجة عن الإصابات الدماغية المكتسبة كالسكتات والرضوض والأورام. وانطلقنا في هذا البحث من إشكاليةٍ مركزية تتبعنا من خلالها مظاهر الخلل المعجمي والدلالي في السياق التخاطبي للمرضى، ومدى تفاوته باختلاف موضع التلف العصبي ونوع الحبسة؛ ولإثبات فرضياتنا، اعتمدنا على التكامل بين المنهج الوصفي التحليلي لتأصيل المفاهيم، والمنهج العيادي القائم على المعاينة المباشرة للحالات، مستعينتين بأدواتٍ سريرية دقيقة شملت المقابلة، واختبارات التسمية، وتحليل الخطاب الحر، واختبارات الفهم. وفي الشق النظري، قاربنا مفهوم الحبسة من زوايا طبية ولسانية وأرطوفونية متكاملة، وصنّفناها إلى أنماطها الإكلينيكية؛ فأبرزنا "حبسة بروكا" التعبيرية التي اتسمت بصعوبة الإنتاج والفقر المعجمي، و"حبسة فيرنيكه" الاستقبالية الطليقة التي تميزت بتدفق لغوي سلس لكنه فاقد للمعنى ومليء بالخلط الدلالي، فضلاً عن "الحبسة التوصيلية" الناجمة عن تضرر القوس المقوس. ومن خلال معالجتنا العيادية للخصائص المعجمية، كشفنا عن صعوبات حادة لدى العينة في استرجاع الكلمات من المعجم الذهني، وتواتر الأخطاء الإبدالية (الصوتية والدلالية)، واللجوء لظاهرة الدوران حول الكلمة بالتعابير الوصفية البديلة. أما على المستوى الدلالي، فقد رصدنا اضطرابات واضحة في اتساق السياق، وخلطاً بين المفردات المنتمية للحقل الدلالي الواحد، وضعفاً في الترابط المفاهيمي العام. وقد فسّرنا هذه النتائج في ضوء أربع مقاربات لسانية: البنيوية، والتداولية، والمعرفية، والعصبية الشبكية التي تربط العجز بتضرر المسارات الدماغية الموزعة. كما أظهرت مناقشتنا للبيانات تأثيراً للمتغيرات الفردية؛ حيث وجدْنا أن كبار السن ومنخفضي التعليم أبدوا فقراً لغوياً أشد، بينما ساهم التعليم العالي في خلق مرونة وقدرة أفضل على التعويض. وتوصلنا في خاتمة بحثنا إلى أن هذه الاضطرابات ليست عشوائية بل هي انعكاس منظم للتلف العصبي، ووضعنا بناءً على ذلك توصياتٍ تدعو لتصميم برامج تأهيل أرطوفونية تركز على التدريب الدلالي والسياقي لإنعاش الشبكات العصبية المتبقية.
Summary :
In our study entitled "Lexical and Semantic Characteristics in the Speech of Aphasia Patients: A Neurolinguistic Study," we examined the interactive relationship between the cognitive language system and the neural structure of the brain. We focused on observing and interpreting linguistic deviations resulting from acquired brain injuries such as strokes, trauma, and tumors. Our research began with a central problem through which we traced the manifestations of lexical and semantic dysfunction in the communicative context of patients, and the extent to which these dysfunctions vary according to the location of the neurological damage and the type of aphasiaTo prove our hypotheses, we relied on an integrated approach combining descriptive-analytical methodology for establishing concepts with a clinical approach based on direct case observation. We employed precise clinical tools, including interviews, naming tests, free speech analysis, and comprehension tests. Theoretically, we approached the concept of aphasia from integrated medical, linguistic, and speech-language perspectives, classifying it into its clinical types. We highlighted expressive "Broca's aphasia," characterized by difficulty in production and limited vocabulary. Wernicke's fluent receptive aphasia, characterized by a smooth but meaningless and semantically confused flow of language, was also observed, as was conductive aphasia resulting from damage to the auditory canal. Our clinical analysis of lexical characteristics revealed significant difficulties in the sample retrieving words from the mental lexicon and a high frequency of substitution errors (both phonetic and semantic). The study also revealed a tendency to use wordplay with alternative descriptive expressions. At the semantic level, we observed clear disruptions in contextual coherence, confusion between vocabulary belonging to the same semantic field, and weakness in overall conceptual cohesion. We interpreted these findings in light of four linguistic approaches: structural, pragmatic, cognitive, and neuro-referential, which links the deficit to damage to distributed brain pathways. Our analysis of the data also demonstrated the influence of individual variables; we found that older adults and those with lower levels of education exhibited greater linguistic deficits, while higher education contributed to greater flexibility and compensatory abilities. In conclusion, we determined that these disruptions are not random but rather It is a systematic reflection of nerve damage, and based on this we have developed recommendations calling for the design of speech therapy rehabilitation programs that focus on semantic and contextual training to revive the remaining neural networks.
